Apply to the traditional ADN program or the LVN to RN Advanced Placement program in 2 easy steps!

Step 1:

1. All students must complete the ADN Multicriteria Evaluation Request form. (The evaluation request form will only be processed during the following evaluation period: February 18^th, 2025, 8:00 a.m. through March 25^th, 2025, by 4:00 p.m.
   
   You will be prompted to log into your Chaffey College Portal and routed to the form once you click on the form link below.

2. Once you receive your official ADN Multicriteria Evaluation form returned from the Evaluation Office via email, use the respective supplemental application documents and application links to apply to the program of your choice.
   
   

Step 2:

Prepare supplemental application documents: Please use the following downloadable supplemental documents for consideration of extra points to be submitted with your ADN application (if applicable). * LVN to RN Advanced Placement applicants do not need to complete these forms for their applications.

• Recent Work/Volunteer Experience Verification form
• Life Experience/Special Circumstances form
• Language Other Than English form
