General Liability
If you wish to file a Claim for Damages against the District, you must submit a Claim Form. Please note, there are various timing requirements regarding the submission of a Claim. If you have questions about relevant statutes of limitation, you should seek advice from legal counsel.
You can obtain a Claim Form in the Office of Human Resources, or at the link below. You must file your Claim Form in person at the Office of Human Resources, or you must submit your Claim Form via mail to the mailing address above.
Be sure to include any relevant information and documentation to support your claim. The District shall act on a claim within 45 days after the Claim Form is received. If the District does not act within 45 days from receipt of the Claim Form, the claim shall be deemed rejected by operation of law on the 45th day.
Claim forms are a public record and may be subject to disclosure pursuant to a Public Records Act request.
Student Insurance
The District offers insurance for injured students and athletes. The policy is excess coverage to any other valid/collectible health insurance plan and provides a maximum annual benefit per injury of $50,000 for students and $25,000 for athletes. To file a claim, follow the procedures in the How to Submit a Claim document below. Please also send a copy of the claim form to Nicole Leonard in Risk Management.
Subpoenas
The law requires personal service of every subpoena. Subpoenas are accepted in the Office of Human Resources Monday through Friday from the hours of 7:30 a.m. to 4:30 p.m. A subpoena to a particular person can only be accepted by that person, and will not be accepted by the District.
