Pursuant to section 76355 of the Education Code, students who can provide documentation of active membership in a religious organization that relies exclusively on prayer for healing may request to have the health services fee waived. Please email the documentation to Jennifer.detisch@chaffey.edu.
This Religious Exemption Request Form needs to be completed each semester.
