Please use this worksheet in consultation with a Chaffey College Counselor. Requirements for admission to the ADN program include the successful completion of courses in biological sciences, along with prerequisites and additional general education courses. All requirements must be fulfilled prior to starting the program.

Preferred consideration for multicriteria screening and ranking will be given to student applicants who have satisfactorily completed ALL ADN prerequisites and general education requirements with the minimum grade point average (GPA) requirements.

Please note: This worksheet and a counselor’s review do not replace the required ADN Multi Criteria evaluation conducted by the Transcript Evaluator Office. Students must request that evaluation by the deadline specified in the ADN application instructions.

All classes listed must be completed with a grade of "C" or better. Students must be in good standing (not on probation). Effective Fall 2014, all plus and minus grades MUST be used in the GPA calculation.
