Chaffey College International Student Office Application Requirements [ Download in .pdf format ] International students are encouraged to apply as far in advance of the desired entry date as possible to allow sufficient time for application processing and other arrangements. Please provide all required documents. Upon receipt of all documents you will be notified by email of your acceptance to the college. Application Forms Below are links to both the Chaffey College Application for Admission and International Student Application forms. Please assure that your application is completely filled out, signed and submitted prior to application deadlines. Application forms may be viewed, downloaded with type option (type in your information), and printed from our web site by clicking on the appropriate links. If you have difficulty with this process, printed forms are available by mail from the International Student Office. Mail Completed application to: Chaffey College Attention: International Student Office 5885 Haven Avenue Rancho Cucamonga, CA 91737 USA Proof of English Proficiency In order to be directly accepted to Chaffey College, international students must show proof of English proficiency. Provide one of the following:
Transcripts Official high school and college/university transcripts with certified English translation. Financial Verification Provide a bank statement or letter from a bank verifying $17,000 is available to cover tuition, fees, and living expenses for one year. The letter should be in English and on official letterhead. If sponsor or parents are in the US, an Affidavit of Support (INS Form I-134) must be completed and submitted. The form is available at the International Student Office or downloaded from http://www.chaffey.edu/international/admission.shtml. Please also note that the bank statement cannot be older then 6 months from the date your file is complete. Copy of Valid Passport A copy of a valid passport that shows name, picture, and date of birth is required. Provide a birth certificate if you do not have a passport. Photograph A passport-size photo should be attached at the top right-hand corner of the International Student Application form (International Student F-1 Visa Information form). On separate sheet of paper type mailing address and email address. Tuberculosis (TB) Test Proof of recent TB test within the past year is required. Note: TB testing is available at the Chaffey College Student Health Center for the fee of $5* (*Subject to change). Students will not be allowed to attend classes without proof of a negative TB test. Medical Insurance **Medical Insurance: ALL STUDENTS MUST PURCHASE MEDICAL INSURANCE FROM CHAFFEY COLLEGE (College Provider: Renaissance Agencies, Inc.) The medical insurance must be purchased prior to the beginning of the each term. The medical insurance can be purchased per each term or for the whole year. **Per year cost $912, fall $380 and spring/summer $532. For more information please contact international student office (rates subject to change). WE DO NOT ACCEPT FOREIGN COUNTRY OR OUTSIDE COMPANY MEDICAL INSURANCE. **The Medical Insurance prices subject to change. Note: Student applying from abroad after receiving all of the above mentioned documents, we will be able to review your application for admission to Chaffey College. We will issue your acceptance letter and other documents as soon as you are admitted to Chaffey College and ready to be mailed or picked-up. |
