Printing and Duplicating Requisition

This form is to be used to request services to be performed by the central duplicating and lithography departments. Please enter information as completely as possible. Space is provided to make special comments.

When submitted, the form will be reviewed by the appropriate staff and processed according to established methods and timelines.
