Transfer form: (Transfer students within the U.S. only) — Must submit a completed transfer form.
Orientation/Activity Fee: An orientation/activity fee of $40 (non-refundable) must be included.
You can pay the fee by cashier's check, money order or credit card (Visa, MasterCard). If you
wish to write personal check, it should be made payable to Chaffey College. If paying by credit
card, please complete the credit card authorization form and mail along with your application.
Transcripts: Official high school and college/university transcripts & diploma with certified
Copy of Valid Passport: A copy of a valid passport is required. Provide a birth certificate if you
do not have a passport. Note: (For Transfer student also include copy of I-94, visa & I-20).
Photograph: A passport-size photograph.
Proof of English Proficiency: International
students must show proof of English proficiency. Provide one of the following:
TOEFL (Test of English as a Foreign Language) minimum score: 450 on the paper based; 45 on
the iBT (Internet based). Chaffey College TOEFL institution code is
4046. Please include this information when taking the test. Or
IELTS (International English Language Testing System) Minimum score: 4.5 or higher or
Transcript indicating 6 years of English instruction or
Intensive English School Completion (letter or certificate)
Financial Verification: Provide a bank statement or letter from a bank verifying $19,000 is
available to cover tuition, fees, and living expenses for one year. The letter should be in English
and on official letterhead. If sponsor or parents are in the US, an Affidavit of Support (Form
I-134) must be completed and submitted. Please also note that the bank statement cannot be
older then 6 months from the date your file is complete.
Tuberculosis (TB) Test: Proof of recent TB test within the past year is required. Note: TB
testing is available at the Chaffey College Student Health Center for the fee of $5* (*Subject to
change). Students will not be allowed to attend classes without proof of a negative TB test
Medical Insurance: Medical insurance must be purchased from Chaffey College prior to the
beginning of each term. The medical insurance can be purchased per each term or for the whole year. Per
year cost $912, fall $380 and spring/summer $532. For more information please contact

Please Note: All international students are required to have health/medical
insurance while attending Chaffey College. The students will purchase the
medical insurance through Chaffey College upon arrival.

Upon review of your completed application and documents, your acceptance letter and other
documents will be issued as soon as you are admitted to Chaffey College. These
documents will be made available to you either via mail or pick-up.
