Hall of Fame Nomination

Athlete Criteria:

Outstanding contribution to the athletic program while a student-athlete at Chaffey.

Completed at least one full sport season as a student-athlete at Chaffey.

Outstanding acheivement in athletics and/or career endeavors.

Has made a contribution to their community through civic or volunteer activities.

Has been gone from Chaffey for five or more years.

Service Criteria:

Community or Chaffey staff member that has worked to improve or enhance the quality of athletic competiton at Chaffey.
The information provided in this form will be sent to the Chaffey College Athletic Hall of Fame Committee.
